My child needs medicine at school

Most prescribed medicines are given three times per day and can be given outside school hours by parents and guardians. Medicine can only be given to a child in school if the dosage is required four times or more per day as prescribed by the child's GP. In that instance, the information in this form will accompany the medicine in school and the instructions laid out in it by you will be followed. It is therefore very important that all fields be fully completed.
